Have you ever noticed how some women with endometriosis seem to have what seems to be like random allergic reactions? They react to everything. Food, heat, stress, perfume, hormonal changes. And at times it seems like you looked at something the wrong way and boom, a flare, skin flushing and more. Let's talk about what may actually be happening. Because for a lot of endo warriors, the missing piece may be mast cells. And when you understand mast cells, mast cell activation syndrome, endometriosis and adenomyosis together, suddenly a lot of the weird symptoms stop looking weird.
They start looking biochemical. Mast cells are immune cells. Think of them like little inflammatory grenades sitting inside your tissues. They live near blood vessels, lymphatic vessels, connective tissue, and nerve endings. And when they sense danger, they degranulate. That means they release chemicals like histamine, tryptase, chymase, prostaglandins, leukotrienes, cytokines, nerve growth factors, and other inflammatory mediators. In a normal situation, this is protective. But in MCAS, those mass cells become unstable. They release too much too often in response to things that should not create a full body reaction.
Now, here's where this connects directly to endometriosis. Endometriosis is not just tissue growing outside the uterus. That explanation is painfully outdated. Endometriosis is a neuro immune inflammatory disease environment. The lesions are not just sitting there, they are communicating. They are recruiting immune cells, they are creating blood vessels, they are remodeling tissue, they are growing nerves, they are generating inflammatory Mediators and according to the mechanisms laid out in the research, mast cells appear to be one of the major orchestrators of this environment in endometriotic lesions, especially ovarian endometriomas and painful fibroid lesions.
Mast cell markers like T P S, A B1 and C P A3 can be elevated because tryptase positive and chymase positive. Mast cells do not just cause allergies. They can degrade tissue, they can remodel extracellular matrix, they can stimulate nerve fibers, they can amplify pain. They help create that angry, inflamed, hypersensitive lesion environment that makes endometriosis feel like your pelvis has been wired into an alarm system. In adenomyosis, mass cells appear to concentrate in transitional lesions near the endometrial myometrial junction, the boundary between the endometrium and the uterine muscle.
One of the most important connections is estrogen. Estrogen is not just telling endometrial like cells to grow. Estrogen can also influence immune behaviour. So estrogen is not just gasoline for the lesion, it may also be gasoline for the immune cells that protect and inflame the lesion. When they degrade, granulate histamine gets released directly onto pain fibers. Histamine then binds to H1 receptors on sensory neurons. That signaling sensitizes TRPV1. Once TRPV1 is sensitized, the nerve threshold drops. Pressure feels like stabbing, bloating feels like tearing, cramps feel like labor, intercourse feels like damage.
A bowel movement feels like passing glass. Once those nerves fire, they release neuropeptides like substance P and CGRP. Substance P can then feedback onto mass cells and trigger even more degradation. So the nerve activates the mass cell and the mass cell activates the nerve. And now you have a self amplifying neuro immune loop. This is how pain becomes chronic. This is how pelvic pain becomes bladder pain. This is how bowel pain becomes back pain. This is how endometriosis becomes a whole body experience.
Your pain being out of proportion to imaging and diagnostics does not mean that your pain is fake. It may mean that the immune system and the nervous systems are amplifying each other. This isn't random. This is a pattern. And once you see the pattern, you stop blaming yourself, you stop thinking you're crazy, and you stop accepting explanations that insult your intelligence. You start to ask better questions. The goal is to understand the machinery creating the symptoms. That is where real authority begins.
