It's not just in your head or your back. Why? Endometriosis and adenomyosis causes leg pain and leg numbness. From that heavy dragging feeling to that weird electric shock down the thigh, that burning sensation, that deep ache that starts in the pelvis and travels down the leg. And the classic response women get is usually maybe it's your back, maybe it's sciatica, maybe you pulled something. That doesn't sound related to endometriosis, but here's the truth. It absolutely can be related. And if we understand the anatomy, the nerves and the inflammatory chemistry, it makes perfect sense.

Endometriosis and adenomyosis do not only cause pelvic pain, they can create pain that travels. Why? Because your pelvis is not isolated. Your pelvis is a nerve hub. It is filled with nerve roots, nerve plexuses, blood vessels, fascia, ligaments, bowel structures, uterine tissue, pelvic floor muscles and inflammatory immune cells. So when endometriosis or adenomyosis creates inflammation in that area, the pain does not always stay where the lesion is. It can radiate, it can refer, it can irritate nerves directly. It can sensitize nerves chemically.

And in more severe cases, lesions can physically involve or compress pelvic nerves. This is why some women feel pain not just in the uterus or pelvis, but in the hips, buttocks, thighs, calves, feet, lower back and even the groin. Now, let's separate the nerves because the location of your leg pain matters. If the pain runs down the back of the leg, especially From the buttock down the posterior thigh, calf or even into the foot, we think about the sciatic nerve. The sciatic nerve comes from the lumbar sacral plexus, mainly L4, L5, S1, S2 and S3.

But in endometriosis, sciatica does not always come from a disc in the spine. Sometimes it can come from pelvic disease irritating the sciatic plexus or sciatic nerve pathway. This is why some women have what's called catamenial sciatica, meaning sciatica like pain that gets worse around menstruation. That is a massive clue because if your leg pain follows your cycle, that is not behaving like a typical pulled muscle. It is behaving like hormonally responsive inflammatory nerve irritation. If the pain is more in the front of the thigh or you feel weakness extending the knee or climbing stairs feels harder, then we think about the femoral nerve.

The femoral nerve comes from the lumbar plexus, mainly L2 to L4. It affects the anterior thigh and the quadriceps. If the pain is in the inner thigh, especially the deep growing area or the medial thigh, then we think about the obturator nerve. The obturator nerve helps control the adductor muscles, the muscles that bring the thigh inward. Now here's the key point. Leg pain and endometriosis can happen in two main ways. One is direct nerve involvement. The second way is through chemical nerve sensitization.

Endometriotic lesions create an inflammatory microenvironment. They release cytokines and prostaglandins that tell pain fibers to fire more easily. Let's talk about NGF, and VEGF. NGF encourages nerves to grow into areas where they should not be overly active. VEGF promotes blood vessel formation. The lesion is not just building blood supply, it is building sensation.
